To participate in Interscholastic Sports, students must be seen by the school nurse for medical clearance prior to each sport season. In order to be medically cleared, the student must have Form A (filled out by your physician or the school physician) and Form B (filled out by parent/guardian) on file with the school nurse. Once cleared the nurse will give the student the medical clearance form to submit to the coach on the first day of tryouts.
Health Examination Form. This form must be either on file in the nurse’s office or brought to the nurse prior to the start of the sport season. It must be dated within one year to the start of the sport season that your child is trying out for. It must be stamped and signed by your physician or the school physician.
Interscholastic Sports Health History Update. This form must be completed and signed by a parent/guardian and brought to the school nurse no earlier than 30 days prior to the start of the sport season. It is complete only if all questions are answered YES or NO. (Any question answered yes must be explained.) The sport your child would like to try out for must be noted on the form.
