Student Registration

In order to register your child, you and your child must be
physically domiciled (living full time and sleeping) at a
house or apartment inside of our district boundaries.

To request a registration link, contact the District Registrar at 516-364-5669 or by email. Office hours are between 9am and 4pm Monday through Friday.

Before you begin, gather the following:

  • Household Information - address and phone numbers
  • Parent Information - work and cell phone numbers, email addresses
  • Student Information - demographic and health/medication information, including current student photo
  • Emergency Contact - important phone numbers
  • Home Language Questionnaire 
  • PDF files of various documents showing proof of Residency, Birth, etc. 

Examples of documents you can upload: Passport, transcript/report card, driver's license, IEP/504 plan, birth certificate, mortgage statement, lease, deed, tax forms, pay stubs, custody papers, student records release form.

Registration applications must be first submitted online. Once it is reviewed, new registrants are required to make an in-person appointment with our District Registrar to provide original documents. The student registration office is located at South Woods Middle School (99 Pell Lane, Syosset). Your registration with the District is not complete until you meet with the Registrar.

Once you've met with the Registrar and your application has been reviewed, you will receive an email regarding submission of health forms

Our Online Registration Application can be displayed in English, Chinese (Simplified), Japanese, Korean, Spanish, Hindi and Urdu. All free-form text on the application must be completed in English, regardless of the “displayed language.” If you need assistance, or require a translator to complete your application, please contact the District Registrar.

If you would like to inquire about or register your Preschool Student for a special education evaluation/services, please call the Committee on Preschool Education Department at 516-364- 5735.
